MYTH: "Document everything."
The correction: document the decisions and exceptions, not the obvious. A 40-step SOP nobody reads is debt, not an asset.
SOP: Deciding what to document
Run this whenever someone says "we should write this down."
Stage 1 — Filter
— ☐ Is this done more than once a quarter? If no, leave it as a ticket. (Owner: process author)
— ☐ Does it break if the wrong person does it? If no, skip. (Owner: process author)
Stage 2 — Scope
— ☐ Write only the steps where people actually get it wrong. (Owner: doc author)
— ☐ Capture the one judgment call that needs a human, name who decides. (Owner: doc author)
Stage 3 — Test
— ☐ Hand it to someone who's never done it; they run it cold. (Owner: reviewer)
— ☐ Fix every step they paused on. (Owner: doc author)
Owner: process author. Trigger: a task repeats and breaks. Done-when: a stranger ran it without asking you a question.
Save this. Run it every time.
