MYTH: "RACI is corporate bureaucracy."
The correction: skipping RACI doesn't remove the roles, it just leaves them unassigned. The real waste is two people both think they're Responsible.

SOP: One-line RACI for any task
Run this whenever a task has more than one person near it. (RACI = who's Responsible, Accountable, Consulted, Informed.)

Stage 1 — Assign
— ☐ Name exactly ONE Accountable. If you can't, the task isn't defined. (Owner: lead)
— ☐ Name who does the work (Responsible). Can be the same person. (Owner: lead)

Stage 2 — Trim
— ☐ List who must be Consulted before, not after. (Owner: lead)
— ☐ List who's just Informed; they get a message, not a vote. (Owner: lead)

Stage 3 — Publish
— ☐ Paste the four names in the task itself, not a separate doc. (Owner: lead)

Owner: lead. Trigger: a task touches two-plus people. Done-when: exactly one Accountable name exists.
