Q: Should I install GA4 with Google Tag Manager or just the gtag.js snippet?

A: Both work, but they suit different teams.

— Use gtag.js (Admin → Data Streams → your stream → View tag instructions) if it's a simple site, you rarely add events, and you don't want a second tool.
— Use Google Tag Manager if you'll add many custom events, need version control, want non-developers to deploy tags, or run multiple pixels.
— Don't run both for the same property — double-firing inflates pageviews. If you migrate to GTM, remove the hardcoded gtag snippet.

TL;DR: gtag for simple, stable setups; GTM once tracking gets complex or many people touch it. Never both at once for one property.
