Short answer: usually a refresh saves it, decay is normal as coverage falls behind. A B2B site had a "remote work tools" pillar that peaked at 9,000 monthly clicks, then slid to 5,200 over a year.
The refresh: re-checked the top results, added 5 newly relevant subtopics competitors now covered, updated stats, and re-linked it from 10 newer related posts.
Result in 7 weeks: clicks recovered to 8,600 and the page reclaimed two lost top-3 positions.
In practice: competitors expand coverage and your page stands still, so it falls behind. Refreshing the entity coverage and re-establishing internal links usually reverses the slide.
