Q: A redirect was killing 22% of our form conversions — how was it caught?
A: A lead site fired its conversion on a /thank-you page, but a marketing redirect sometimes sent users to /thank-you/ (trailing slash) which had no tag:

DebugView: replayed a real submission and saw no conversion on the slash variant

Found two URL versions in Engagement → Pages and screens

Moved the conversion trigger from the URL match to the form's submit event instead
After switching from page-URL tracking to event-based tracking, reported form conversions jumped 22% overnight — those leads were always real, just untracked.
TL;DR: URL-match conversions break on redirects and trailing slashes. Trigger on the actual submit event and recover the silent losses.
