Run the operation off a single recurring report, not memory.
1 — Every Monday, pull counts: new dead links found, qualified, in-outreach, placed, monitoring.
2 — Compute week-over-week deltas for each stage.
3 — Trigger: any stage flat or down two weeks running? Open a root-cause line item.
4 — Flag the oldest item stuck in "in-outreach" past 14 days for close-out.
5 — QA gate: numbers must reconcile, total in equals total moved plus carryover.

Definition of done: one reconciled snapshot per week with deltas, stuck-item flags, and a root-cause line for any stalled stage.
