If you've genuinely decided to disavow, build the file like this — sloppy files cause more damage than the links.

— Use domain: entries, not individual URLs. One bad domain spawns hundreds of URLs.
— One domain per line, lowercase, no http://, no trailing slash.
— Add comment lines with # dating each batch and why. Future-you needs the audit trail.
— Never disavow a domain that also sends you good links — it's all-or-nothing per domain.
— Keep the master file in version control. The uploaded file replaces the old one entirely.
— Re-upload the full file every time; partial uploads wipe prior entries.
