Asking "any feedback?" once a year gets noise. Run a fixed listening loop so problems surface before members quietly leave.
☐ Weekly: one low-friction temperature poll ("how's the pace this week?")
☐ Monthly: one open prompt — "what's one thing we should change?"
☐ On exit: auto-DM departing members one question ("what made you leave?")
☐ Log every theme in a feedback doc, tagged by frequency
☐ Close the loop publicly: "you said X, we did Y"
The SOP's non-negotiable step is the last one — feedback you never act on visibly trains members to stop giving it. Exit feedback is the most valuable and most ignored; one question at the door beats ten in a survey nobody opens.
Review themes monthly; ship one visible change from the list.
