HTML tags failed most often — hand-edited per page, they drift; one missing reciprocal breaks the cluster.
So what: sitemap-delivered hreflang centralizes the matrix in one generated file, so reciprocity is programmatic, not per-page hope. It also keeps page weight down on 40-locale sites. Use HTML tags only for small, stable language sets. At scale, generate hreflang from one source of truth into the sitemap — the error rate roughly halves, and clusters stop silently breaking on deploys.
