Meet a med spa in Miami. Beautiful reviews, ranked well for "med spa," invisible for "botox" and "laser hair removal" — the searches that actually paid.
The owner had set her primary Google Business Profile category to "Medical Spa" and stopped there. One category, doing all the work, and it didn't match how people searched for her highest-value services.
The instinct was to stuff every related category in. We didn't. Primary category carries the most ranking weight by far — that's the one big decision. We kept "Medical Spa" as primary but added precise secondaries: Skin Care Clinic, Laser Hair Removal Service, Facial Spa.
Three weeks, flat. Then "laser hair removal near me" started surfacing her listing.
Two months later she was in the pack for two service searches she'd never appeared for, and those were her best-margin treatments. Bookings for laser up 40 percent.
The lesson: primary category is your single heaviest ranking lever — choose it for your money service, not your generic label. Secondaries widen the net, but they never outrank a well-chosen primary. Most listings leave that one decision on default.
