Got a policy strike, here's how I write the appeal (when one's allowed):
— State the EXACT page/URL you fixed, don't be vague
— List what you changed, bullet form: 'removed X, added Y, noindexed Z'
— No excuses, no 'I think Google is wrong' — they don't care
— Confirm you re-reviewed the whole site, not just the flagged page
— Keep it under 150 words, reviewers skim
My reinstated appeals were all short + specific. Long sob stories got auto-denied.
